Stress and poor sleep have become the default state for most working Indians. A 2023 survey by the Indian Psychiatry Society found that India ranks among the most stressed countries in the world, with chronic workplace stress, financial anxiety, and disrupted sleep affecting a significant majority of urban adults. The supplement market has responded with hundreds of ashwagandha products making bold claims about stress relief and sleep improvement. But does ashwagandha actually work, and if so, which form works best?
This post covers the clinical evidence for ashwagandha's effects on stress and sleep, how it works mechanistically, and what you need to know to choose an effective product.
What is Ashwagandha and Why Does It Help with Stress?
Ashwagandha (Withania somnifera) is a small shrub native to India, North Africa, and the Mediterranean. It has been used in Ayurvedic medicine for over 3000 years as a rasayana, a rejuvenating tonic for strength, vitality, and longevity. Its Sanskrit name means "smell of horse," referring both to its distinctive odour and to the traditional belief that it confers the strength and vitality of a horse.
Ashwagandha is classified as an adaptogen, a substance that helps the body adapt to physical and psychological stress by modulating the stress response rather than suppressing it. Unlike sedatives or anxiolytics that force the nervous system into a calmer state, adaptogens work by supporting the hypothalamic-pituitary-adrenal axis, the hormonal communication system that governs the body's response to stress.
The primary active compounds in ashwagandha are withanolides, a class of steroidal lactones concentrated in the root. Withanolides are responsible for most of ashwagandha's adaptogenic effects. The concentration of withanolides varies enormously between different ashwagandha products, which is why standardisation matters so much when choosing a supplement.
How Ashwagandha Reduces Cortisol
Cortisol is the primary stress hormone produced by the adrenal glands in response to physical or psychological stress. In short bursts cortisol is useful, it sharpens focus, mobilises energy, and prepares the body for action. But chronically elevated cortisol, which is the state most stressed urban adults live in, causes a cascade of problems including disrupted sleep, impaired memory and cognitive function, weight gain particularly around the abdomen, reduced immune function, hormonal imbalance, and accelerated cellular ageing.
Ashwagandha reduces cortisol through two primary mechanisms. First, withanolides inhibit the activity of the enzyme that converts cholesterol into cortisol in the adrenal glands, reducing the raw production of cortisol at the source. Second, ashwagandha supports the sensitivity of glucocorticoid receptors, the cellular receptors that cortisol binds to, allowing the body's natural cortisol regulation feedback loop to function more effectively.
The result is lower baseline cortisol levels, a more proportionate stress response, and faster recovery to baseline after stressful events.
What the Clinical Research Says
KSM-66 Ashwagandha is the most clinically studied form of ashwagandha available, with over 24 published human clinical trials. Here is what the research specifically shows for stress and sleep.
Cortisol reduction and stress relief. A double-blind, randomised, placebo-controlled study published in the Indian Journal of Psychological Medicine enrolled 64 adults with a history of chronic stress. Participants taking 300mg of KSM-66 twice daily for 60 days showed a significant reduction in serum cortisol levels and scored considerably lower on the Perceived Stress Scale, the General Health Questionnaire, and the Depression Anxiety Stress Scale compared to the placebo group. The researchers concluded that KSM-66 safely and effectively improves an individual's resistance towards stress.
Sleep quality and duration. A randomised, double-blind, placebo-controlled study published in Cureus enrolled 150 healthy adults and individuals with insomnia. Participants taking 120mg of KSM-66 daily for six weeks showed significant improvements in sleep onset latency, meaning they fell asleep faster, total sleep time, sleep efficiency, and morning alertness compared to placebo. Participants with insomnia showed greater improvements than healthy adults, suggesting KSM-66 is particularly beneficial for those with existing sleep difficulties.
Anxiety reduction. A randomised controlled trial published in Medicine found that participants taking KSM-66 showed significant reductions in anxiety scores on the Hamilton Anxiety Rating Scale after eight weeks compared to placebo. The effect size was clinically meaningful, not just statistically significant.
Cognitive function. A study published in the Journal of Dietary Supplements found that KSM-66 supplementation significantly improved immediate memory, general memory, executive function, sustained attention, and information processing speed in healthy adults after eight weeks. This is particularly relevant for stressed individuals whose cognitive performance is impaired by chronically elevated cortisol.
Thyroid function. A pilot study published in the Journal of Alternative and Complementary Medicine found that KSM-66 supplementation in patients with subclinical hypothyroidism significantly improved TSH, T3, and T4 levels after eight weeks. Thyroid dysfunction is both a cause and consequence of chronic stress, making this a meaningful finding for stressed individuals with unexplained fatigue.
KSM-66 vs Regular Ashwagandha for Stress and Sleep
This is the most practically important question for anyone buying an ashwagandha supplement in India, where the market is flooded with cheap generic products.
Generic ashwagandha extracts use a mixture of root and leaf material, have no standardised withanolide concentration, have no clinical trial data behind the specific extract, and frequently use chemical solvents in extraction. When you buy a generic ashwagandha capsule for Rs. 200, you have no way of knowing how much withanolide content it contains or whether it will produce any measurable effect.

Buying generic ashwagandha to save money is effectively buying a product with no reliable clinical backing. The cost difference between a generic product and a KSM-66 product is real, but so is the difference in efficacy.
Ashwagandha and Sleep: The Mechanism
Ashwagandha's effect on sleep is not simply a consequence of reduced stress, though that plays a role. It also works through a more direct neurological mechanism.
Withanolides in ashwagandha have been found to bind to GABA receptors in the brain, the same receptors targeted by sleep medications and anti-anxiety drugs, but through a gentler, non-sedating mechanism that supports natural sleep architecture rather than forcing sedation. This is why ashwagandha improves sleep quality and morning alertness rather than simply making you feel drowsy.
The compound triethylene glycol, found in ashwagandha leaves, has also been identified as contributing to sleep induction in animal studies. KSM-66, which uses root only, contains withanolides as the primary active compounds rather than triethylene glycol, making its sleep effects primarily GABA-mediated.
When and How to Take Ashwagandha for Stress and Sleep
Timing matters more for ashwagandha than for most supplements. Here are the two most evidence-informed approaches.
For stress and cognitive function as the primary goal, take ashwagandha in the morning with breakfast. This supports cortisol regulation throughout the day when stress is most likely to occur and provides cognitive benefits during working hours.
For sleep as the primary goal, take ashwagandha in the evening one to two hours before bed. This allows the GABA-modulating effects to peak during the sleep onset window and supports deeper, more restorative sleep through the night.
For both stress and sleep, splitting the dose morning and evening as used in the primary cortisol reduction study produces the most comprehensive effect.
Ashwagandha works cumulatively. Most people notice improved stress resilience and sleep quality within two to four weeks of consistent daily use. The full benefits for cortisol reduction and cognitive function typically emerge after six to eight weeks. Taking it occasionally produces no meaningful effect.
Who Should Not Take Ashwagandha
Ashwagandha is considered safe for most healthy adults at standard doses. However it is not appropriate for everyone. Pregnant women should avoid ashwagandha as it may stimulate uterine contractions. People with autoimmune conditions including rheumatoid arthritis, lupus, and multiple sclerosis should consult a doctor before using ashwagandha as its immune-modulating effects may exacerbate these conditions. People taking thyroid medication should monitor thyroid levels when starting ashwagandha given its documented effect on thyroid hormone levels. People scheduled for surgery should stop ashwagandha at least two weeks before the procedure as it may interact with anaesthesia.
